Please read  and follow these steps carefully to be fully registered.
IMPORTANT:  You are only registered once ALL steps are completed

Athletics teaches many valuable lifelong lessons and requires a high level of dedication, responsibility, and diligence.  

Please pay special attention to all the enclosed information. It is imperative that all physical paperwork is accurately completed, signed and submitted to the School Nurse in a timely manner to ensure school doctor approval and eligibility for sports.  Athletic Physicals must be current within 365 days.  If you are not sure of the date of your last physical contact the School Nurse or the Athletic Trainer’s Office 

SITUATION 1: If the physical on file IS NOT dated within 365 days of the first day of practice you will need a new physical and must complete an entire Full Physical Packet including the subsequent forms.
SITUATION 2: If the physical on file IS dated within 365 days you ONLY need the Health History Update Form and subsequent forms must be completed in order for your student athlete to participate.

IF YOU HAVE ASTHMA OR ANAPHYLAXIS (Epi-Pen), YOU MUST SUBMIT YOUR UPDATED TREATMENT PLAN FORM FROM YOUR DOCTOR EVERY YEAR.

ANY INJURY NOTED IN THE HEALTH HISTORY UPDATE FORM REQUIRES CLEARANCE FROM YOUR PHYSICIAN IN ORDER TO COMPETE AND MUST CONTAIN THE DATE OF THE INJURY.
